Scheduling :: Appointment Express If you have been referred by your dentist for the extraction of teeth you may be able to schedule your extraction without a consultation appointment. This will save you the extra time of having to wait for a consultation appointment and the time between your consultation and the extraction appointment. There are two things we will need in order to schedule your surgery.

1. A referral from your dentist and the radiograph if your dentist is providing the x-ray. If your dentist does not have adequate radiographs they can be taken at our office but this may require a separate visit to the office prior to your surgery.

2.Completed pre-registration forms (HIPPA Consent, Insurance and Demographic information, Health History). These forms can be completed online and submitted instantly to our office by clicking here. Please call our office if you have any difficulty with online registration. If you prefer, you may print out and complete the forms and either bring them to our office or send them by US Mail.
