:: Financial Arrangements
For
our patients who do not have Medical
and/or Dental insurance:
Payment in full is required at the
time of surgery.
For our patients who have Medical
and/or Dental insurance:
Payment of your out of pocket expense
(co-payment) is required at the
time of surgery.
Upon your request, our office
will make every attempt to verify
your insurance coverage and provide
you with an estimate as to how
much of the total fee will be covered
by insurance. The responsible party
will be required to pay that portion
of the total fee that is likely
not covered by insurance at the
time of surgery. If a balance remains
on your account after your insurance
company (companies) has forwarded
payment to our office, you will
receive a final statement and final
payment of the entire balance will
be due within 10 days.
It is your responsibility to know
your insurance benefits. We will
pre-determine your treatment plan
upon request; pre-treatment determinations
will be honored for 90 days unless
otherwise restricted by your insurance
plan. It is your responsibility
to know if your insurance company
requires pre-determination or pre-authorization
prior to treatment.
As a courtesy to you, we will
send your insurance carrier a claim
for services rendered. It is important
to note that Ohio law dictates
that your insurance company must
pay the claim or respond to the
claim within 30 days. Insurance
companies may deliberately or accidentally
delay the processing of this claim.
We will wait 60 days for their
payment; if it has not been received
within 60 days, you will be expected
to pay the balance in full at that
time. We accept Visa, Master Card,
Discover, and Care Credit for your
convenience.
Estimated out of pocket expense
is based upon information provided
by you or by your insurance carrier(s).
This payment balance represents
that portion of the charges that
the insurance carrier(s) will not
pay and is your responsibility.
Estimates are based on percentages
of a maximum covered fee, or may
be based upon a flat fee accepted
by the insurance company. This “maximum
covered fee” is often referred
to as UCR or maximum allowable
fee. This fee is generated by the
insurance company and may not be
representative of the fees charged
by this office, or by other offices
in this area. You, the patient,
are responsible for all charges
not covered by your insurance carrier(s).
Care Credit is a financing system that many of our patients find helpful for their oral surgery services. You can select from various financing options, some of which allow for extended terms, "same as cash". You may receive more information and apply for Care Credit while visiting our office.
