The Forms area of this website has some important information forms that you will need to use for registering with our office. If you have registered online through the Submit Patient Forms website, you will have supplied us with the most important information. If you would rather print out your forms and complete them prior to your appointment, you may find the necessary forms here. The three most important forms are the HIPPA consent form, the Patient Information form, and the Medical & Dental History forms.

The pre-op and post-op instruction forms contain important and valuable information regarding your treatment and recovery. The pre-op instructions are particularly important if you are going to have sedation for your surgery. After your surgery, you will receive a printed copy of the post-op instructions. The website form found here serves as a back-up should you misplace your printed copy.

Consent forms are a necessary step in the pre-surgical process. In addition to serving as a tool to inform you of potential problems associated with oral surgery, they serve a good stimulus for further questions and sharing of information between you and our office. As you review consent forms, be sure to note any questions or items that need further explanation. Our desire is to have you be as well informed about your condition and treatment as possible.
