Timothy M. Lawrence Appointments for surgical treatment are typically made after a consultation. However, there are some procedures that can be scheduled without a consultation. In order to make your appointment for surgery without a consultation we will need the following:

· A referral from your dentist or physician
· A completed health history form
· A radiograph if your procedure will be the extraction of a tooth
· The necessary information to predetermine your insurance benefit
· Your preference for anesthesia

You may also download the Patient Information and Medical & Dental History forms from this website. If you choose to download the forms and fill them out you will need to bring them or mail them to our office.
