Scheduling :: Surgery Appointments Appointments for surgical treatment are typically made after a consultation. However, there are some procedures that can be scheduled without a consultation. In order to make your appointment for surgery without a consultation we will need the following:

· A referral from your dentist or physician
· A completed health history form
· A radiograph if your procedure will be the extraction of a tooth
· The necessary information to predetermine your insurance benefit
· Your preference for anesthesia

You can submit your insurance information and health history form online by going to the Submit Patient Forms website. You may also download the Patient Information and Medical & Dental History forms from this website. If you choose to download the forms and fill them out you will need to bring them or mail them to our office.
